Palliative Care Knowledge Exchange For Health Directors, HCC Coordinators, Nurses (RNs, LPNs, & NPs), Program Staff & Provincial Partners The Home and Community Care Team in partnership with Lakehead University, Centre for Education and Research on Aging, the Palliative Care Knowledge Exchange will bring together content experts including: clinical providers, decision makers, funders, palliative researchers and community members. In addition to various presentations it will present the five-phase community capacity development model. This model includes local community development, regional partnerships, education and community empowerment. It will then present guidelines and resources for palliative care program development. This two-day Knowledge Exchange will bring together communities and partners with a goal to improve the end-of-life care in four First Nations communities. through developing palliative care programs and creating a culturally appropriate guide to palliative care programming.
